Participation Waiver

Liability Waiver and Terms of Service for Participation in any Tennis Connection Activity

1) Photograph Authorization
I understand that my photograph, and/or my child’s (person participating in tennis activity), may be taken during his/her participation in tennis or any group tennis activities with the Tennis Connection and I authorize Tennis Connection permission to display said photos, if taken.

2) Release of Liability
I / We hereby RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE The Tennis Connection of West Florida, Inc. dba Tennis Connection and any and all employees and independent contractors from any liability, claims, demands, actions, and causes whatsoever arising out of or related to any loss, damage or injury, including death, that may be sustained by myself or my child / legal ward, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEE, or otherwise, while participating in athletic and tennis activities, or while in, on or upon the premises where the activities are being conducted or travel to and from Tennis Connection tennis activity.

By continuing with this registration, you are confirming you've read the "Release of Liability" and "Photograph Authorization" listed above and that you agree to them in order to participate in any Tennis Connection activities.
